Referral Form
Professionals and loved ones can use this secure form to begin the transition process for an adult into our supportive sober living environment. We will ask for fundamental contact details and primary safety considerations to ensure a safe transition. Once submitted, our team will promptly follow up to discuss individual needs, fit, and the next steps in their recovery journey.
Submitting a referral does not guarantee program admission. All referrals are reviewed based on eligibility, program requirements, and housing availability.
What happens next?
- We review your referral within 1–2 business days.
- We contact the individual (or referring professional) to discuss eligibility.
- If appropriate, we schedule an intake interview.
- We determine placement based on program eligibility and availability.
- If we cannot meet the individual's needs, we will provide referrals to other community resources whenever possible.

Generally a good fit
- Adults (18+) committed to a sober lifestyle.
- Individuals capable of managing activities of daily living independently.
- Ready and willing to participate in a shared, community-based home.
- Medically stable and engaged in an outpatient program or working.
- Open to following house community agreements and safety protocols.
Not the right fit right now
- Individuals in need of acute medical detox or 24/7 clinical supervision.
- Those actively experiencing severe psychiatric crisis or instability.
- History of behavior that poses a direct threat to the safety of others.
- Inability to live harmoniously in a shared residential environment.
- Currently unwilling to abstain from all mood-altering substances.
